چکیده

Choledocholithiasis is a problem of remarkable clinical importance, as demonstrated by its incidence, clinical manifestations, and outcomes with sometimes severe complications; nevertheless, great variabilities still exist in the diagnostic and therapeutic distances 1. 10% of the female population and 6% of the male population are affected by cholelithiasis. We know that the lithiasis of the common bile duct (CBD) is associated with gallstones in 10-20 % of patients, with the percentage of association between 8% and 15% in patients under the age of 60 and between 15% and 60% in patients over the age of 60 2. Choledocholithiasis figures most commonly like a complication of gallstones, due to stones moving to the cystic duct from the gallbladder. Choledocholithiasis is a significant source of morbidity, and an optimal program of treatment must preview their identification and removal (pre-, intra-, or postoperatively). In our geographic area, lithiasis of the gallbladder is mostly due to the formation of cholesterol stones 1. We define symptomatic patients as those for whom the cholelithiasis has caused or causes biliary pain (a severe pain in the right hypochondrium or in the epigastric area of a duration of at least half an hour) or complications directly associated with the lithiasis (cholestatic jaundice, acute pancreatitis, or acute cholePervenuto in Redazione Novembre 2010. گزارش 100 مورد لاپارسکوپی

100 patients (93 females and 7 males) in ages ranging between 23 and 65 years were subjected to laparoscopic cholecystectomy for biliary colic (92 pts), acute cholecystitis (7 pts) or asymptomatic gallstones (1 pt). None had a history of icterus and in every case choledocholithiasis was excluded by means of serum alkaline phosphatase assessment and sonography (In addition to ERCP in 3 pts). 3 o...

Treatment of hepatic hydatid disease complications using endoscopic retrograde cholangiopancreatography procedures.

BACKGROUND Liver hydatidosis may lead to serious morbidity due to biliary complications, the management for which endoscopic sphincterotomy (ES) and biliary drainage are very efficient. We evaluated the effectiveness of endoscopic treatment for complications of hepatic hydatid disease. METHODS We retrospectively reviewed endoscopic retrograde cholangiopancreatography (ERCP) procedures perform...

Efficacy and safety of limited endoscopic sphincterotomy before self-expandable metal stent insertion for malignant biliary obstruction

AIM To evaluate the safety and efficacy of limited endoscopic sphincterotomy (ES) before placement of self-expandable metal stent (SEMS). METHODS This was a retrospective analysis of 244 consecutive patients with unresectable malignant biliary obstruction, who underwent placement of SEMSs following limited ES from December 2008 to February 2015. The diagnosis of malignant biliary obstruction ...

A retrospective analysis of endoscopic treatment outcomes in patients with postoperative bile leakage

OBJECTIVE Bile leakage, while rare, can be a complication seen after cholecystectomy. It may also occur after hepatic or biliary surgical procedures. Etiology may be underlying pathology or surgical complication. Endoscopic retrograde cholangiopancreatography (ERCP) can play major role in diagnosis and treatment of bile leakage. Present study was a retrospective analysis of outcomes of ERCP pro...

Endoscopic sphincterotomy with or without cholecystectomy for choledocholithiasis in high-risk surgical patients: a decision analysis.

BACKGROUND Laparoscopic cholecystectomy (LC) is recommended for patients with choledocholithiasis after ERCP with sphincterotomy (ES) and stone extraction. AIM We designed a decision model to address whether ES alone versus ES followed by LC (ES + LC) is the optimal treatment in high-risk patients with choledocholithiasis. METHODS Our cohort were patients with obstructive jaundice who have ...
